Missing a class

Fill this in and we will do the rest. Classes run every week, including school holidays, so if a week is missing here it is worth double-checking before you tick it.
The one hard rule: it has to be in before the class starts. That is what tells the coach not to expect your child, which is what keeps the class right on the day. If we do not hear from you and your child does not turn up, that class is used.
